Rick has more beard than end of last season. Leads me to think that the group spends all summer in rail car A. At some point their captors can no longer contain them and our band of fearless zombie worriers see their moment of escape. Having read the graphic novels I can only surmise that they escape only to be thrust into a new terror of some sort.
I’m pretty sure he is about to get his hand cut off.
I’m guessing that there is some sort of shootout, outside and Rick is checking the coast is clear….

This photo truly gives me some very deep insight on the events of season 5. You can see the pain, the loss in Rick’s eyes. His face is spattered with blood, fresh from killing the unjust. His hands have just strangled a victim of Terminus. The others are dead. Carl is dead. Judith is dead. He has lost all, but he still stands. (Just kidding this is the least revealing image I’ve ever seen)
